Offering a full range of clinical services, from rehabilitation, physical therapy, and disease specific treatments, to palliative and hospice care, this hospital and health system has proven itself a valuable part of the community.
Reporting to the VP Patient Care / CNO, the this Nursing Director will play a vital role in the operations of a Home Hospice Service line. You'll take clinical and administrative oversight of a compassionate and tightknit team of Home Health RNs, Geriatric Registered Nurses and LVNs, Social Workers, ad Wound Care RNs, as well as Physical, Occupational, and Speech Language Therapists. You'll also get to work with leaders from across the organization and from across borders to develop a clinical continuum of care that will improve the lives of patients on a daily basis.
This is an exciting time to join the leadership team at this esteemed organization, because the Home Palliative Care Director will get to play an integral role in the development and growth of this hospice and home care program. With new community partners and a wider scope than before, this RN Director will work with the Area Finance and Operations team develop an actionable budget, as well as a financial maintenance plan to ensure your targets are met. Working in collaboration with the Regional Director of Quality and Compliance, you'll create both a performance evaluation plan and a quality assurance program, building these factors into the bedrock of your Home Health and Hospice Programs.
